LIMITED DENTAL WARRANTY FAQ

Shock Doctor product purchased from Shock Doctor or an authorized seller is warranted to the original owner against factory defects in materials and workmanship for one year after the date your purchased the mouthguard. If a product fails due to a manufacturing or materials defect, then Shock Doctor will replace the product or otherwise make it right.
This limited warranty does not include product failures resulting misuse, improper care, general damage, alterations, modifications or normal wear and tear.
Any alteration of Shock Doctor brand products will void warranty coverage.
Because Shock Doctor cannot control the quality of products sold by unauthorized sellers, this warranty applies only to products that were purchased from Shock Doctor or a Shock Doctor authorized seller in the United States, unless otherwise prohibited by law. Shock Doctor reserves the right to reject warranty claims for products purchased from unauthorized sellers, including unauthorized Internet sites or marketplace storefronts. -
Shock Doctor warrants that the enclosed mouthguard is free from defects in material and workmanship that could cause injury to the purchaser’s sound, natural teeth.
This limited warranty covers dental injuries sustained while using a properly fitted Shock Doctor mouthguard according to the instructions provided during a supervised
athletic contest or coach-supervised training session. -
Yes, our warranty department has to physically inspect the item in order to process the claim. ANY ITEMS SUBMITTED FOR WARRANTY THAT ARE NOT CLEAN OR ARE OTHERWISE DEEMED TO BE UNSANITARY WILL BE RETURNED.

This warranty is made in lieu of all other express warranties. Except as is otherwise expressly provided herein, SHOCK DOCTOR DISCLAIMS ALL LIABILITY FOR LOSS, INJURY
OR DAMAGE BASED UPON NEGLIGENT DESIGN OR DEFECTS IN MATERIALS AND WORKMANSHIP FOR THE WITHIN MOUTHGUARD. Shock Doctor is not responsible for any
indirect, incidental, special, or consequential damages, including but not limited to lost wages, pain and suffering, or additional medical expenses beyond the coverage
outlined above, except where such exclusions are prohibited by law.

To obtain reimbursement of qualifying dental expenses, you must:
i. Submit a complete written notice of the injury or damage to Shock Doctor within thirty (30) days of the injury;
ii. Provide a statement from a licensed dentist verifying the necessary dental treatment received. This treatment must have occurred within thirty (30) days after
the date of injury.
iii. Provide a statement from an official, coach, or superintendent verifying that the injury occurred during a supervised contest or a practice session and that the
mouthguard was in proper use at the time of injury.
iv. Return the mouthguard to Shock Doctor with the written notice of injury. -
This limited warranty does not provide payment or indemnity for:
• Orthodontic treatments, including braces or retainers.
• Preparation of a tooth as an anchor for a bridge.
• Any injury that occurs while the mouthguard was not properly fitted in accordance with the provided instructions.
• Incidental or consequential damages, except where prohibited by law. -
We cannot accept warranty claims from outside of the United States.
REPLACEMENTS AS PROVIDED UNDER THIS WARRANTY ARE THE EXCLUSIVE REMEDY OF THE CONSUMER. SHOCK DOCTOR SHALL NOT BE LIABLE FOR ANY INCIDENTAL OR CONSEQUENTIAL DAMAGES FOR BREACH OF ANY EXPRESS OR IMPLIED WARRANTY ON THIS PRODUCT. Some States do not allow the exclusion or limitation of incidental or consequential damages, so the above limitation or exclusion may not apply to you. Any implied warranties that may be applicable to products, including implied warranties of merchantability or fitness for a particular purpose, are limited in duration to the duration of this warranty. Some States do not allow limitations on how long an implied warranty lasts, so the above limitation may not apply to you.
